Skilled Nursing

Skilled nursing facilities, also called nursing homes or convalescent centers, provide housing and 24-hour skilled nursing services to individuals requiring post-acute care due to decreased ability to perform activities of daily living.

Specialized programs and features vary in our facilities, but many offer fine dining, concierge and valet services, Beautician services, fun and divers activities, on-site physicians and state-of-the-art technologies.

Short-term Rehabilitation

At Monument Health, we understand the need to return our patient’s health back to normal as quickly as possible. Our approach to rehabilitation focuses on the key aim for successful healthcare outcomes. We recognize that unexpected illness or trauma, or anticipated surgeries can sometimes force individuals to relearn basic functions such as standing, swallowing, gripping or walking. Monument Health offers in-house rehabilitation services in each facility, specializing in Physical, Occupational, and Speech Therapy in order to assist patients in regaining their health.

Physical therapy strengthens each patient’s independence and reinforces functional mobility skills following an acute or chronic disease process. This is accomplished through individual care and treatment plans by applying customized interventions.

Occupational Therapy

Occupational Therapy

Occupational therapists work with our patients to achieve independence in activities of daily living. Therapists can help patients refine their motor skills, hand and upper extremity function, and bladder and bowel control for incontinence.
